Question: Many students have asked about the practical clinical skills exam, can you please tell us briefly about each session and the material we should study so they can prepare themselves and study well for it. Also, someone asked if the systolic and diastolic murmurs are included in the exam or not and do we have to memorize – for now – the tables in the examination part where we diagnose what we find on the patient ?

Answer: I did give a revision lecture where I told students all about the exam. I recommend for next year that all students come to the revision lecture!!
There will be a room for doing consultation skills (talking to a simulated patient) – this will be either taking the history for a respiratory OR cardiovascular and the patient will be a bit upset OR angry. You need to take the history (find out why they have come, ask about symptoms, do the systems review (either respiratory or cardiovascular), then there are just a few marks for patient’s perspective, past medical history, drug history / allergies, family history and social history. The last few do not have many marks so you only need to ask the questions briefly.)
Then you will go into a room for physical examination – it will be either respiratory OR cardiovascular OR cervical and epitrochlear lymph nodes. You need talk like it is a real patient (if it is a manikin) for example, “hello my name is… are you Mr Ahmed, can I examine your respiratory system, do you have any pain…” then follow the steps that are listed the handbooks that I have written for the course. You do not need to say “why” you are looking for something but you do need to say “what” you are looking for. Eg – I am looking at the hands for clubbing, splinter haemorrhages, Janeway lesions, Osler’s nodes etc.
Then you will go into a room for clinical procedures – it will be either advanced life support with defibrillation OR venepuncture OR giving a drug IV OR endotracheal intubation OR putting in a cannula.
Again you need to say what you are doing as you do it and don’t forget to talk to the manikin as if it is a real person eg ” I’m …. are you Mr Ahmed? I need to give a drug into your vein, is that OK? Do you have any allergies?” and then say ” I would also check the name and allergies on the prescription chart and wristband”.
So for your practical exam you do not need to know causes, For your theory you will need to know causes, eg splinter haemorrhages are because of infective endocarditis. FOr cardiac murmurs I expect you to know the location and type of murmur and if there is a heave or thrill with it, for aortic stenosis and regurgitation and for mitral stenosis and regurgitation.
